✅ Personal details - address, phone number, etc. 

✅ Your medicare number, including your child’s

✅ Your GP’s details 

✅ Your private health cover if relevant

✅ Dietary requirements and/or plans if needed

✅ Child’s immunisation record 

✅ Any medical forms, plans, diagnosis

✅ Child’s routine

✅ Any court orders in place 

✅ Emergency contacts AND their photos - note this does not have to be their IDs, just their photos and details
